​​New clients

Please print and fill out the new patient information form and bring to your appointment. We would appreciate you arriving 15 minutes prior to your appointment time to fill out intake paperwork. If you are unable to print the new patient information form, it will be available in our office. 

​Rates 

Our rates range from $130 to $200 per hour depending on the service being provided and the length of time. Specific fee schedules per clinician are included in the therapist's Disclosure and Informed Consent form, which will be provided to you during your intake.

The rates for intern therapists are $40 per session and $50 for intakes. 

Some of our therapists do accept sliding scale if you are uninsured. Please ask if that would help you get the services you need. Fees will be determined on a case by case basis. It may also be helpful to see an intern therapist.

We require at least 24 hours advance notice for cancellations. Appointments cancelled less than 24 hours will incur a late cancellation fee.  

Insurance

Please call your insurance provider to make sure the therapist you want to see is covered under your specific plan and what your out of network cost will be if they are not listed as in-network.
